- Elevated serum creatinine of 3.5 mg/dL, suggesting significant kidney dysfunction.
- Potassium level of 5.0 mEq/L, requiring close monitoring for potential cardiac complications.
- Oxygen requirement maintained at 40% FiO₂.
- Dependence on low-dose Noradrenaline (Norad) infusion to maintain adequate blood pressure.
Although the patient was stable enough for transfer, the combination of respiratory failure, cardiovascular disease, renal impairment, and vasopressor support placed the patient in a high-risk category for air transport.
Preparing for a High-Risk Air Ambulance Mission
Transporting a ventilated patient by air is comparable to shifting an ICU from one hospital to another.
Before take-off, our critical care team carefully reviewed the patient's ventilator settings, medication infusions, laboratory reports, and hemodynamic status. Every detail was planned in advance to ensure uninterrupted intensive care during the flight.
Backup oxygen supplies, emergency medications, infusion pumps, portable ventilators, suction equipment, and advanced monitoring systems were checked thoroughly before departure.

Critical Care at Every Altitude
During the flight, our medical team's responsibility extended far beyond monitoring a screen.
The patient's ventilator settings were continuously assessed to maintain adequate oxygenation on 40% FiO₂, while blood pressure was closely monitored because of the ongoing Noradrenaline infusion. Maintaining stable circulation during air transport is especially important, as even small physiological changes can significantly affect critically ill patients.
The patient's cardiac history, renal dysfunction, and electrolyte imbalance required constant vigilance throughout the journey. Every parameter—from oxygen saturation and blood pressure to heart rate and neurological status—was monitored to detect even the earliest signs of deterioration.
For critically ill patients, safe transport depends not only on responding to emergencies but on preventing them before they occur.
Another Mission Completed Safely
The patient was successfully transferred from Jalgaon to Mumbai and safely handed over to the receiving critical care team for further management.
