NICU Train Ambulance From Patna to Sir Ganga Ram Hospital, Delhi

Neonatal transfers involving complex congenital heart disease demand precise temperature control, ventilatory support, and continuous infusion management. Life Savers Ambulance Services executed a highly specialized NICU train ambulance transfer to ensure safe relocation for advanced cardiac care.
Case Overview
A 12-day-old neonate was shifted from Patna to Sir Ganga Ram Hospital, Old Rajender Nagar, Delhi using a NICU-enabled Train Ambulance Service arranged by Life Savers Ambulance Services. The transfer was conducted as a bed-to-bed train ambulance transport, maintaining uninterrupted neonatal intensive care throughout the journey.
Patient Condition
The neonate was diagnosed with Transposition of the Great Arteries (TGA), a critical congenital heart defect requiring urgent surgical evaluation. At the time of transfer:
- The baby was on Prostaglandin infusion to maintain ductal patency
- Developed respiratory depression secondary to prostaglandin, requiring ventilator support
- Required multiple continuous infusions, including:
- Prostaglandin
- Fentanyl
- Adrenaline
- Was clinically fragile and dependent on full NICU-level support
Transport Decision
A Train Ambulance Service in India was selected to enable:
- Continuous administration of critical infusions
- Stable ventilatory support over long distance
- Controlled thermal environment using an incubator
- Space for a complete neonatal ICU setupLife Savers designed a dedicated NICU system within the train ambulance, replicating hospital-level neonatal care during transit.
NICU Setup Onboard
The NICU Train Ambulance arranged by Life Savers included:
- Neonatal ventilator support for assisted respiration
- Transport incubator for continuous temperature regulation
- Infusion pumps for prostaglandin, fentanyl, and adrenaline
- Multiparameter monitoring for heart rate, oxygen saturation, and perfusion
- Backup oxygen and emergency neonatal resuscitation equipment
A specialized neonatal team ensured continuous monitoring and intervention readiness.
Care During Transit
During the Patna to Delhi journey:
- Ventilator support was maintained without interruption
- Infusions were carefully titrated as per clinical need
- The incubator ensured stable thermal conditions throughout
- Vital parameters were continuously monitored
- Despite the complexity of the condition, the neonate remained stable during the transfer.
Arrival and Handover
On arrival at Sir Ganga Ram Hospital, Delhi, the baby was safely handed over to the NICU team for immediate continuation of advanced cardiac management and surgical planning.
Clinical Perspective
Neonates with TGA on prostaglandin infusion are highly vulnerable to respiratory compromise and require integrated ventilatory, pharmacological, and thermal support during transport. Establishing a functional NICU environment within a train ambulance enables safe long-distance transfer for such high-risk cases.
Life Savers Ambulance Services has successfully operationalized NICU Train Ambulance transport in India, enabling the movement of critically ill neonates with continuous incubator and ventilatory support over long distances. This capability reflects advanced planning, specialized equipment integration, and trained neonatal teams, ensuring continuity of life-saving care until definitive treatment can be provided.
