Intercity air transfer of neonates with complex congenital heart disease requires meticulous planning, regulatory coordination, and continuous intensive monitoring. Life Savers Ambulance Services managed a medically supervised commercial air transfer for a critically ill newborn requiring advanced pediatric cardiac care.

Case Overview

A 12-day-old infant was shifted from Imphal to Kolkata for urgent evaluation and management at a higher cardiac center. The transfer was executed via a commercial air medical transport, coordinated and supervised by Life Savers Ambulance Services, with full medical escort and ICU backup.

Patient Condition

The neonate was diagnosed with multiple complex congenital cardiac anomalies:

  • Ventricular Septal Defect (VSD)
  • Transposition of the Great Arteries (TGA)
  • Pulmonary Atresia

This combination of defects significantly compromises oxygenated blood flow, placing the infant at high risk of hypoxemia, circulatory instability, and rapid clinical deterioration. Given the age of only 12 days, the physiological reserve was extremely limited.

Transport Decision

A commercial air ambulance model with medical escort was selected due to:

The medical escort team carried a semi ICU neonatal backup, including:

  • Portable monitoring for heart rate, oxygen saturation, and perfusion
  • Emergency airway and resuscitation equipment
  • Essential cardiac and hemodynamic medications

The infant was accompanied by a critical care doctor and a trained paramedic, both experienced in neonatal stabilization and in-flight monitoring.

Execution and Monitoring

During the Imphal to Kolkata flight:

  • Oxygenation and perfusion were continuously monitored
  • Hemodynamic parameters remained stable
  • The infant tolerated cabin altitude changes without acute desaturation episodes
  • Emergency preparedness protocols remained active throughout the journey

The coordinated airport handling minimized movement stress and ensured safe boarding and disembarkation.

Arrival and Handover

On arrival in Kolkata, the neonate was safely transferred to the receiving pediatric cardiac unit. A structured clinical handover was provided, detailing cardiac status, oxygen needs, and in-flight observations to ensure seamless continuation of care.

Neonates with VSD, TGA, and pulmonary atresia represent some of the most complex congenital cardiac cases in pediatric medicine. Air transfer in such scenarios demands:
